Please use this form to request issuance of your digital microcredential badge. If you are pursuing more than one microcredential and expect to complete them at the same time, you may use a single form – check all that apply.
This form must be completed to be awarded a microcredential, even if you are currently enrolled in one (or more). Microcredentials are awarded after final grades are processed and completion of microcredential requirements have been verified by the designated department and certified by the Registrar’s Office. You will receive an email with information on claiming your digital badge via Credly, and your microcredential(s) will be included on your official SUNY Broome transcript.
